You will be working for our local GP to support our residents in their home.
As an Advanced Nurse Practitioner (ANP), you will use your expert knowledge and complex decision-making skills, guided by the current NMC Code in all situations, accountable for the total episode of care for patients with undifferentiated and undiagnosed needs.
You will be responsible for assessing patients of all ages, and attending the GP surgery with undiagnosed illness and injury. Initiating treatment and discharging or referring to the appropriate care pathway as needed.
Working autonomously at an advanced practice level as well as within a multidisciplinary team, using a person-centred, partnership and self-directed approach, developing and sustaining new partnerships and networks to influence and improve health, outcomes, and healthcare delivery systems.
You will be a passionate Registered Nurse first level on the NMC register, with a Graduate Level qualification, 3 years post registration experience in Primary, Community, Urgent or Acute Settings and hold the NMC non-medical prescribing qualification V300 (or be willing to undertake this programme).
